Role of LMWH in Recurrent Pregnancy Loss – Insights from CREATE in ART 2025
On 2nd August 2025, I had the honour of presenting at CREATE in ART 2025, a prestigious conference hosted by the Institute of Reproductive Medicine (IRM) in Kolkata. The event commemorated the 97th birth anniversary of Prof. B.N. Chakravarty, a pioneer in Indian reproductive medicine.
Why Recurrent Pregnancy Loss (RPL) Matters
RPL — defined as the loss of two or more consecutive pregnancies — is a devastating condition for couples. The causes are varied: genetic, anatomical, hormonal, immunological, and sometimes, unknown. One proposed contributing factor is microthrombosis (tiny clots) in the placental circulation, which can impair fetal development.Where LMWH Fits In
Low Molecular Weight Heparin (LMWH) is an anticoagulant that prevents abnormal clot formation. Beyond its blood-thinning effect, LMWH also appears to influence implantation and placental development by:• Enhancing uterine blood flow
• Modulating immune activity
• Supporting trophoblast invasion (key for implantation)


Evidence & Clinical Application
Research shows mixed results — LMWH is not a universal solution for all RPL patients. However, in women with:
• Documented thrombophilia
• Antiphospholipid antibody syndrome
• Certain unexplained RPL cases with suspected clotting issues
LMWH, often combined with low-dose aspirin, can improve pregnancy outcomes.
My Key Message
Treatment must be individualized. LMWH should be considered only after a thorough evaluation, ensuring the benefits outweigh risks. Overuse without clear indication is not advisable.
